Immunization Requirements

All incoming, degree-seeking, undergraduate 91勛圖厙 students are required to meet the American College Health Association (ACHA) immunization recommendations prior to moving into campus housing or starting classes, whichever is earlier.

Required vaccines (based upon ACHA guidance):

  • Hepatitis B series- three doses
  • Measles, Mumps, Rubella (MMR) - two doses (first dose on or after 1st birthday) or blood test showing immunity
  • Meningitis conjugate vaccine (MCV) - on or after 16th birthday
  • Tdap - on or after 10th birthday and required within the past ten years
  • Varicella - two doses (first dose on or after 1st birthday) or blood test showing immunity

Additional recommended vaccines (no documentation is required and no exemption request is needed; the University reserves the right to move any of these to required vaccines if ACHA updates its recommendations):

  • COVID-19 vaccine series
  • Influenza vaccine- annual influenza vaccine
  • Hepatitis A series
  • HPV series
  • Meningitis B series
  • Polio (OPV or IPV) series

If you have a medical contraindication to any of 91勛圖厙's immunization requirements, please have your medical provider complete this form. One form per vaccine exemption request. Please upload the completed form(s) to . Health Services staff will review and follow up with you.

Please note that 91勛圖厙 does not accept requests for exemption from undergraduate immunization requirements on the basis of religious objections. As a religiously affiliated institution with extensive external academic placements we have a responsibility to provide a safe living and learning environment for our undergraduate students and the broader community. In fact, before COVID-19, 91勛圖厙 experienced a mumps outbreak that resulted in the termination of hundreds of external student placements due to the rapid spread of the disease among undergraduate students.
